For individuals who are ready to live more independently, we provide safe, welcoming homes that provide staff support by experienced, caring Direct Support Professionals (DSP). With apartments and single-family homes nestled in the neighborhoods of Park Ridge and Des Plaines, Illinois, our residents get a private space to call their own, within the community.
Community Integrated Living Arrangements (CILA)
Our state-licensed CILA homes are designed for eight or fewer adults with intellectual and developmental disabilities to get the support they need to live safely and independently. Direct support staff are available around the clock, and each home meets strict standards for care, privacy, and community living.

Avenues’ homes are intentionally located within walkable neighborhoods. Residents can run errands, meet friends, go to work, and build real connections – experiences that strengthen confidence and a sense of belonging.
Independence at your pace
At Avenues, support is never one-size-fits-all. Each resident receives personalized support from dedicated staff. We provide support with daily routines, medications, community engagement, healthy living, and more, while also teaching essential skills such as cooking, housekeeping, and budgeting. As capabilities grow and needs change, support adjusts accordingly, allowing residents to build independence at a pace that feels right for them.
Beyond home
Life at Avenues extends well beyond the front door. Residents are part of a community that includes day services, creative and social activities, community employment services, health support, and regular outings. Whether it’s building skills, making friends, or exploring new interests, our residents have access to programs that help them stay active, connected, and growing.
Our homes
Avenues’ homes are located in beautiful neighborhoods offering a stable, comfortable environment where adults with developmental disabilities can build independence. Residents have their own private bedrooms, giving them personal space and a sense of ownership, along with shared living areas. Our homes are supported by caring, experienced staff. We work closely with residents and their families to match each person with the home that best fits their needs and preferences, ensuring they live in a setting designed to help them grow with confidence and dignity.
