For adults who remain in their family home or live independently with HBSS through the Illinois Department of Human Services, Avenues offers support for navigating home-based services with Self-Direction Assistance (SDA). Our team primarily supports families in and around the Northwest suburbs of Chicago.
What is self-direction assistance (SDA)?
Self-Direction Assistance (SDA) is a key support available through Illinois’ home-based services offering. SDA provides practical support and guidance so you can independently plan, direct, and manage your HBSS funding. Avenues’ experienced SDA staff offer an ongoing partnership that helps:
- Arrange and coordinate services with providers
- Direct personal support workers and services
- Coordinate billing and service hours
- Understand and manage your budget and service plan
- Access supports such as therapies, assistive devices, and home modifications
