In addition to our core services, Avenues offers a variety of additional services that help the individuals we serve stay healthy, engaged, and connected.
Feeling good, living well
Our Clinical Services Department supports the ongoing health and wellness of individuals enrolled in our residential and day programs. With guidance from registered nurses and close collaboration with staff, we focus on proactive care, daily routines, and habits that support long-term wellbeing. Registered nurse services are available weekly – primarily for individuals in our homes – and staff help reinforce healthy living through cooking activities, movement, and everyday wellness practices.
Clinical services include:
- Coordinating with doctors, pharmacies, and specialists
- Medication management
- Health monitoring, dementia screenings, and chronic condition support
- Encouraging healthy routines and activities of daily living (ADLs)
Discovering new interests
Life at Avenues includes plenty of opportunities to have fun, discover new interests, and build relationships. From art, music, and cooking to fitness classes and games, activities are built into the weekly routine at our homes and our Center for Independence.
Individuals also enjoy seasonal celebrations, picnics, special events, and community outings to local attractions, and some choose travel excursions through the Hornbrook Adventure Program. Whether someone prefers quiet creative time or lively group events, there’s always something fun within reach.
Making sense of government benefits
People with disabilities and their families spend much of their lives navigating eligibilty for various government benefits. We offer comprehensive benefits management for residents of our CILA homes: maintaining eligibility for Medicaid, SNAP, Social Security/SSI, and Medicare. With 20+ years experience in this area, our Benefits and Program Supports Manager can provide valuable advice and assistance to other Avenues participants and their families.
