Do I need funding in place to apply?
Not always, but it depends on the service you’re interested in. We can help you understand what funding or eligibility is required.
What if I’m not sure which service is the best fit?
That’s common. Start with the interest form or contact us — we’ll help you figure out the right next step.
Can I tour one of your locations or see services in action?
Yes! We can discuss what’s possible based on the service you’re exploring.
Do you provide services for children?
Avenues provides services for adults. If you’re planning ahead for a child who will be transitioning to adult services, we can discuss timing and options.
How long does the process take?
Timing varies based on the service and availability. Our team will talk through realistic expectations with you after we learn more.
What are home-based services/SDA and do you provide caregivers?
Home-based services/SDA supports people who live at home or independently with waiver funding. Avenues does not provide hands-on caregiving; we support families in coordinating services, budgeting, and navigating the program.
Where do you offer your services?
We proudly serve the North and Northwest sides of Chicago, along with the surrounding North and Northwest suburbs.
