Care Coordinator (Direct Support Specialist DSP)
Qualifications:
- 4 year degree in social services or related field or;
- High school diploma and ability to pass Department of Human Services mandated training program. Passing all required Illinois Healthcare Workers Background Checks.
Mission:
The Care Coordinator is responsible for coordinating all medical care for residents and communicating health related information to the IDT. The Care Coordinator plays an essential role in ensuring residents receive excellent medical care within the timeframes indicated by DHS.
Example of Responsibilities:
- Promote the health and well being of the clients we serve; Administers medication under Rule 116
- Assist residents in accessing medical care, coordinating medical appointments, and scheduling all necessary follow up appointments.
- Communicate all medical information to the residents IDT. Maintain an appointment calendar that is accessible to all Avenues IDT members.
- Completes all appropriate medical documentation accurately and in a timely manner.
- Assist persons served in skills of daily living, personal care, social adjustment, homemaking, budgeting, integration and independence;
- Develops, implements and monitors individual service plans for persons served;
- Participates in IDT and other meetings as necessary;
- Insures safe environment for all persons served;
- Develops and maintains natural community supports, fostering relationships with non-paid persons in the community.
- Provides supportive counseling on an on-going basis and at time of crisis;
- Advocates on behalf of persons served;
- Assists individuals to access transportation including rides to and from work;
- Participates in seminars or other training sessions for the purpose of career growth and development;
- Perform other duties as required or assigned
