Avenues supports community living for more than 70 adults. Our residents, whether living In our apartments, townhomes, or single family homes, are encouraged to experience everyday life in their community.

They receive training in the areas of cooking, housekeeping, budgeting, and other daily living skills that are essential for independent living. These and other decision making skills are carried out by each individual to the best of their capabilities, and as independently as possible.

Staff provides support depending upon the specific needs of each resident. Some require round-the-clock care, while others might only need a short, daily check-in.

Residential & Group Homes

Our Community Living Facility (CLF) offers adults an opportunity for semi-independent living with staff members on the premises 24 hours a day to assist and train individuals in various daily living skills.

The Supported Living Arrangement (SLA) is a residence designed for individuals who can manage daily living with only intermittent staff support, varying from 20 to 40 hours of support per week.

The Community Integrated Living Arrangement (CILA) program offers more customized support services including skills training, personal assistance and employment related services.  All services are designed to meet each individual's needs, preferences, and strengths regardless of abiliites.  All of the CILA programs at Avenues provide 24 hour staff on premises support.

Based on the needs of each person served, the residential programs will provide, arrange, or refer the individual for support or training in the following areas:
  • Basic academic skills
  • Basic self-care
  • Communication skills
  • Consumer affairs
  • Contingency planning, problem solving and decision making
  • Financial management
  • Health maintenance
  • Housekeeping and home maintenance
  • Human sexuality
  • Interpersonal relationships
  • Management of personal and legal affairs
  • Menu planning and meal preparation
  • Mobility and community transportation skills
  • Recreational and leisure time activities
  • Safety practices including dealing with injuries and life threatening emergencies
  • Self advocacy and assertiveness training
  • Telephone usage
  • Utilization of community services and resources
  • Work attitudes and skill exploration
  • Life issues and transitions
The skill areas are taught through goals or classes with various program staff.  Daily activities reflect the needs and choices of the persons serviced.

Avenues Home Based Support Services

The home based support services (HBSS) program is a fee-for-service program funded by the Illinois Department of Human Services.  This program is tailored to meet individuals’ evolving needs and preferences for supports allowing them to continue living in their own home.

Individuals and their guardians may choose the particular array of separate HBS services, up to a monthly dollar allotment set by the Illinois Department of Human Services, which best meets the needs of the individual. All services provided must be for the direct benefit of the individual and must be directly related to their disability. Home Based Support Services is an option within the adult Medicaid waiver.

Each individual enrolled in the HBSS program must have an agency associated Service Facilitator.  Avenues’ Home Based Service Facilitator works with families to develop an appropriate individual program plan that will best meet the needs and desires of the individual.  In addition, they coordinate billing, hours and assure individuals receive the identified services in their service plan.

Some of the services provided under the HBSS program include:
  • Personal Support Workers
  •  Developmental Training, Adults Day Care and Supported Employment
  •  Nursing
  •  Training and Counseling for Unpaid Caregivers
  •  Behavioral Intervention and Behavior Counseling
  • Adaptive Equipment, Assistive Technology, Vehicle Modifications and Home Accessibility Modifications
  • Occupational, Physical and Speech Therapy
Avenues currently works with 25 families in the HBSS program.


The Illinois State system is currently limited in its acceptance of individual Support Options. Under its Home-based model, funding and access are restricted due to funding constraints.
 
Statewide, efforts are underway to expand the Home-based Individual Support Options model and to possibly make this the predominant funding mechanism for future programs.

Avenues is pleased to be working with families to develop individualized Support Programs wherever possible.
